General Liability Waiver
I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
In consideration of my ticket purchase and permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
(A) I WAIVE, RELEASE, AND DISCHARGE Fox Haven Farm & Retreat Center. from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: Fox Haven Farm & Retreat Center. and/or their founders, contractors, volunteers, representatives, and agents, and the activity holders
(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this statement from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
Educational Instruction & Herbal Medicine Waiver of Acknowledgement

Accordingly, if you have any medical or health issues, we strongly recommend that you consult with your physician or medical health care provider with regard to any medical conditions or prior to implementing herbal medicine procedures into your health regimen.
By signing below, you agree that neither Fox Haven Farm & Retreat Center. nor any of its employees, contractors, practitioners, co-founders, shareholders or agents shall have any liability to you under any theory of liability in connection with the use of information, services, and/or products provided to you by Fox Haven Farm & Retreat Center.
This agreement shall be binding upon your heirs, personal representatives and executors. Notwithstanding the foregoing, in the event that any of the foregoing provisions are held by a court of competent jurisdiction to be invalid or unenforceable, you agree that the total liability of Fox Haven Farm & Retreat Center. if any, for losses or damages shall not exceed the amount paid by you for the particular information, services and/or products provided to you.
By signing below, you are requesting and consenting to the education and utilization of the principles and techniques taught by Fox Haven Farm & Retreat Center.
Photo Release
I hereby grant the Fox Haven Farm & Retreat Center. permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.
I understand and agree that all photos will become the property of the Fox Haven Farm & Retreat Center. and will not be returned.
I hereby irrevocably authorize the Fox Haven Farm & Retreat Center. to edit, alter, copy, exhibit, publish or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.
I hereby hold harmless, release, and forever discharge Fox Haven Farm & Retreat Center. from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR SIGNATURES BELOW. I ACCEPT:
Food & Drink Waiver & Release of Liability
I acknowledge that I may be consuming food and drink from various establishments and sources while taking part in the Activity, and I understand that Fox Haven Farm & Retreat Center. makes no guarantees, representations or warranties, whether express or implied, regarding any food or drink I may consume. Any ill effect from any food or drink, including but not limited to food poisoning, intoxication, sickness, or allergic reaction shall not be the fault or responsibility of Fox Haven Farm & Retreat Center. I expressly and voluntarily assume any risk related to the consumption of food or drink during the Activity.
Allergy Statement
Menu items and/or kitchen surfaces may contain or come into contact with WHEAT, EGGS, PEANUTS, TREE NUTS, and MILK. If you have food allergies, please notify Fox Haven staff immediately.
COVID Protocol
Please do not come to class if you are feeling ill or have been exposed to people in your life who are sick. Please contact alecks@foxhavenfarm.org so we can discuss how to handle your absence.
With the new variant spreading rapidly, we ask all participants to wear a mask both indoors and outdoors. Please do your best to keep 6 feet social distance at all times. If you learn that you have COVID within 5 days after class, we ask that your please notify us so that we can inform participants about a possible exposure.
